These Terms of Service ("Terms") govern your use of Clinical Encounter Notes ("the Service"), a clinical documentation tool operated by Ylunga Consulting. By creating an account or using the Service, you agree to these Terms.
Clinical Encounter Notes is a web-based tool that uses AI (via Anthropic's Claude API) to assist clinicians and Community Support Workers in generating clinical session notes from recorded transcripts. The Service provides note drafts in standard clinical formats (SIRP, SOAP, DAP, BIRP, GIRP, Progress, and Case notes).
The Service is intended for use by licensed or practicing clinicians, therapists, counselors, and Community Support Workers. By using the Service, you represent that you are a qualified professional or are working under the supervision of one.

All notes generated by the Service are drafts intended as a documentation aid only. The AI may produce inaccurate, incomplete, or inappropriate content. You, as the clinician or worker, bear full responsibility for reviewing, editing, and approving any note before it becomes part of an official record. We accept no liability for clinical decisions made based on AI-generated content.
Your use of the Service is also governed by our Privacy Policy. By using the Service, you consent to the collection and processing of data as described therein, including the transmission of session transcripts to Anthropic's Claude API for note generation.
You retain ownership of all clinical notes and transcripts you create using the Service. The Service, its design, branding, and underlying code remain the property of Ylunga Consulting.
The Service is provided "as is" without warranties of any kind, express or implied. To the maximum extent permitted by law, Ylunga Consulting shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of the Service, including but not limited to clinical outcomes, data loss, or service interruptions.
We reserve the right to suspend or terminate accounts that violate these Terms, engage in misuse, or fail to maintain an active subscription. You may delete your account at any time by contacting us.
We may update these Terms from time to time. Continued use of the Service after changes constitutes acceptance of the new Terms. We will notify registered users of material changes via email.
